Health Information Form

Please print out and fill out this Health Information Form. We recommend you post it on your refrigerator door in an envelope with a large Red Cross. This medical information makes it easy if EMTs have to transport you to the hospital.  Every member of your family living at home should have their own form filled out. It is also imperative that you update this info at least once per year.

We also recommend you have a lock box installed by your front door – NOT the one with a combination lock but one with a keypad. You should provide the code to your local police department who can then provide it to the 911 operators. Then if there is an emergency and you are not able to physically let the first responders into your house they can used the lock box to enter your house. Police and/or EMTs and/or firemen will not tear down your door to gain entry.

Substance Disorder and Addiction Support Group 

At the Catholic/Lutheran Response to the Opioid Crisis Seminar, the expression for this disorder is “a homesickness for God.” My hope is that we will find compassion to accept, without judgement, that this is a brain disease that could impact anyone we hold dear. May we open our hearts to those who face the stigma, suffering and isolation of this devastating disease.

Open meetings to provide resources, offer support for those struggling with this disorder, those in recovery, and their families and friends are held each Tuesday from 6:30-8:00 PM in Si Lounge.

The Substance Disorder and Addiction Support Group

  1. Meets on a weekly basis
  2. Is led by its members, who share their personal stories to help one  another solve problems.
  3. Relies on every member’s participation, which can involve talking, listening, or silent gestures of encouragement.
  4. Advises participants to team up with a veteran member to be their sponsor: a mentor who can provide individual support and guidance whenever it’s needed.
  5. Faith based, but non-religion specific.
  6. Promotes the 12 step program
